ACCESS—short for Advancing Chronic Care with Effective, Scalable Solutions—is a ten-year program structured to encourage health outcomes rather than simply tallying clinical activities. Participating organizations like Pair Team receive predictable payments to manage chronic conditions, but they only earn full reimbursement when patients achieve measurable health goals, such as improved blood pressure or reduced chronic pain. The program covers essential areas like diabetes, hypertension, and mental health issues.
A Shift in the Payment Model
The shift in payment structure is groundbreaking. Traditional Medicare reimburses based on time spent with a clinician, neglecting the contributions of AI systems that monitor patients and engage them between visits. While ACCESS opens new avenues, Batlivala expresses caution regarding some participants in this initiative. “I’m a big fan of wearables, but for a senior struggling with food insecurity, I’m not sure how effective those devices can be,” he remarked. Pair Team has tailored its services for a distinct demographic: individuals managing chronic conditions while facing challenges like unstable housing or food insecurity—an estimated one-third of the American population.

Pair Team launched in 2019, emphasizing a holistic approach to healthcare. It combines medical, behavioral, and social care for vulnerable populations, generating results supported by peer-reviewed research. Batlivala shared a poignant moment when Flora had a lengthy conversation with a 67-year-old woman living in her car, calling it “both incredible and depressing.” These interactions highlight how AI can provide emotional support and meaningful connections.

Batlivala believes the low reimbursement rates can be beneficial for organizations that effectively harness AI. “If you want a model that incentivizes AI use, the reimbursement has to be low,” he explained. Pair Team has already secured partnerships allowing access to around 500,000 potential patients, aiming for a target of one million in the next three years.
